I agree with D. The questions get tricky sometimes with the end, I think thats really the trick to beating this exam. This particular question asks "What should they have done?" Not "What should they do" or "what should they do next" The issue is that neither of them know how to operate it. If this was a design flaw that made it over-complicated, that would be an issue where the team could get with operations and train them, but the issue here is that its such a complex software by nature that the team can build it but not know how to operate it/how things interact with each other. A was recommended, but having the team meet with operations through any point in the project is usually a net negative for both the team and operations, and in this case, meeting won't do anything because the team also doesn't know how to work the software.
